I, the undersigned, do hereby voluntarily submit my application, as well as the application for any minors of whom I am a legal guardian, for participation at Colossal Fight Company, LC (henceforth known as “Colossal Fight Company”, “Colossal”, or "CFC"). I acknowledge that there are possible risks of injury involved with this participation. I hereby waive and release all claims, causes of action, losses, damages, costs, expenses, either known or unknown, now or arising in the future, that I or my charges may have of any kind or nature against any instructor, student, or anyone in any way involved with CFC. I understand that the course offered is high-impact and state that I and my charges are free from illness or ailment that may be adversely affected from such activity. I further agree that any media (pictures, videos, etc.) taken may be used for promotion or publicity without any compensation.

Sickness/Illness: I understand that sicknesses of all types are transmissible during activities such as those provided by CFC. If I or my charges are exhibiting any signs of illness, or have knowingly been exposed to an illness, it is my responsibility to not attend class until those symptoms or signs of illness have passed or I am reasonably certain that they/we are healthy. If I or my charges are exposed to and/or receive an illness, I agree to hold blameless all instructors, students, or anyone in any way involved with CFC.

This waiver supersedes all previously signed waivers with Colossal Fight Company.
